Incident Reporting Procedures for DPS Solution Pool

1.    Contact Tiara Hunt, Solution Pool Project Manager immediately by phone at 

2.    Complete the incident report form (below) 

3.    DPS will coordinate work related injury care if required 

4.    Employee will report within 24 hours of incident to DPS offices for incident investigation reporting and remediation if required 

5.    If required, Houston Methodist site manager will be asked to participate in investigation, including but not limited to interviews with clinical nurse liaison and providing test results on source patient 

6.    DPS will provide updates to the HMH Staffing Management Team as well as site managers on employee status, assignment coverage, and incident conclusion.
